Situation Brief
What is reported
- WHO Weekly Epidemiological Record (25 Sep 2026): 2,646 confirmed plague cases, 3,847 suspected and 423 deaths for 2019–2025.
- Case fatality is 11% among suspected cases (423 of 3,847).
- DR Congo and Madagascar account for 97.7% of confirmed cases.
- Russia: a 28-year-old Irkutsk Anti-Plague Institute technician died on Oct 1–2 after hospitalization on Sep 29. Rospotrebnadzor says the cause was pneumonia of unknown etiology and that tests found no microorganisms linked to her work.
- About 200 contacts were placed under observation in Irkutsk. They are contacts, not cases: tests found 2 COVID-19 and 2 rhinovirus infections and no plague, and none showed illness linked to infectious disease. Monitoring is nearing completion and a local official says about 60% have been released (Oct 6).
- WHO (Geneva, Oct 6) says Russia told it there were no plague cases in the city of Irkutsk. Reuters could not establish whether this rules out plague in the technician's own case.
- The independent outlet People of Baikal (relayed by RFE/RL, CBS and Axios) reports a lab accident with plague bacteria on Sep 25. Russian authorities deny any accident, and no source has confirmed plague.
- The US State Department and CDC are monitoring the case and have asked Russia to share information quickly and openly. President Trump offered help; the Kremlin welcomed the offer without saying whether it will accept.
Awaiting verification
- Laboratory confirmation or exclusion of Yersinia pestis in the Irkutsk death
- Why official Russian statements conflict with local and independent reports
- Whether a lab accident occurred, and whether anyone else was exposed
- Whether any onward human-to-human transmission has occurred (none reported)

As of Oct 6 Rospotrebnadzor says monitoring is nearing completion and tests found 2 COVID-19 and 2 rhinovirus cases, no plague, and no illness linked to infectious disease among contacts.
